FBI Identity History Summary Check FD-258, FD-1164
- The FD-258 This file card (blue) is used for civil submissions to the FBI pertaining to individuals applying for criminal justice positions
- The FD-1164 is used for fingerprinting for identity history summary checks.

Online Criminal Record Check
1- Standard Level 1 - CRC/CPIC:
- Check of the National Repository of Criminal Records based on a person’s name and date of birth.
- It may also include searches of other national and local databases.
- This check is usually required for Private Employment (Security Guard License , Uber, Lyft, retail, trades, administration, manufacturing, construction, and management) .
- It provides criminal convictions from CPIC and /or local police records.
- This check does not require fingerprints.

2- Enhanced Level 2 -CRCJM:
- A review of an individual’s criminal history, as well as other relevant information such as outstanding charges convictions, pardons, and non-conviction information that may be relevant to an employment where the individual may have a high level of authority, trust, or responsibility.
- This check does not require fingerprints.

Identification requirements
TWO PIECES OF GOVERNMENT ISSUED ID ARE REQUIRED FOR RCMP RECORDS
Please also bring with you, the instructions that you have received from the requesting organization to perform the fingerprints.
Important
- Only original IDs are accepted as Primary ID
- One of the two ID should have a photo and signature
- Certified copies can only be accepted as a secondary ID
- SIN card is not an acceptable ID
- Credit Card is not an acceptable ID
